Ventricular tachycardia in Arrhythmogenic Right Ventricular Cardiomyopathy: How to Manage?

Arrhythmogenic Right Ventricular Cardiomyopathy (ARVC) is a condition characterized by fibrofatty replacement of the RV myocardium due to genetic abnormality. Structural changes may be absent or minor in early stages disease and localized specific region RV. Clinically it appears as electrical instability. To reduce risk arrhythmic events sudden cardiac death, device therapy pharmacotherapy recommended. In this paper, we describe case female with ARVC brief discussion based on literature review. The patient presented chest discomfort accompanied palpitation. An electrocardiogram (ECG) showed ventricular tachycardia apex origin, convert symmetric inverted T-waves probable epsilon waves right precordial leads, mimicking pseudo-right bundle branch block (RBBB) pattern following cardioversion. parasternal short-axis view echocardiography shows severe dilatation. Subsequent workup using CMR was planned but refused. We diagnosed background suspicious arrhythmogenic cardiomyopathy. were able provide accurate diagnosis treatment, avoiding potentially fatal consequences. As result, critical recognize potential ECG findings know when pursue further research implement therapies.
